Estate Planning Documents

Estate planning documents allow a person to outline their medical treatment preferences and where their property will go after death. The documents are recommended for individuals reaching seniority (55+) and allow family members to make medical and financial decisions if they cannot do so for themselves.

Do I need an attorney?

Although it is recommended, an individual does not need an attorney to complete their estate plan.

Advance Directive – Combines a medical power of attorney and living will into 1 form. Allows an individual to choose their medical representative while also setting up their treatment preferences.

Durable Power of Attorney – Grants the power of a person (principal) to choose someone else (agent) and make financial decisions on their behalf.

Last Will and Testament – Allows a person (decedent) to decide how their assets will be distributed after their death.

Living Will – Outlines the health care treatment preferences for an individual in the event they cannot speak for themself. Such options include artificial breathing and feeding if in a health condition with no possibility of a cure.

Medical Power of Attorney – Lets a patient choose someone else (agent) to make health care decisions on their behalf and in accordance with their wishes.
